#1004 - Field Not Displaying In Quick Create Form (Subpanels)

Hi there,

I've installed everything successfully, however I cannot see the field when I try to upload the document from any quick create field in the subpanels -- this is something that is vitally needed so any help you can provide would be most welcome.
