Will this work out of the box on a system that has a lot of custom modules? Or do you need to do extra work

Yes, SecuritySuite works with custom modules out of the box. Note: if a custom module was built by hand instead of with Module Builder then it may require some additional steps to add SecuritySuite support to those specific modules, but usually this is rare. Help is available if that is the case.
If there are issues, usually it's just a matter of editing the vardefs of that custom module and removing the securitygroups reference at the bottom of the file.
