Apologies for the Inconvenience.
Can you please Uninstall the Dynamic Form Addon from the Module Loader and Download the Latest zip from the SuiteCRM Store which we have updated Today. It's fix your issue.
After Downloading the Latest Zip, Try to Install and Do Repair & Rebuild.
