I had a similar issue as case 2555 ... "Database failure. Please refer to suitecrm.log for details."

I went through the full form creation process and received this error each time. Did a Quick Repair several times. Any guidance would be helpful ... last log entries:

    Hello,

    Thank you for using our add-on. It seems database table is not created during installation. Can you please try Quick Repair & rebuild from admin? and do execute query for new table definition. Let us know how it goes.

    Thanks for the quick response!

    I have done the Quick Repair & rebuild from admin several times. I also closed all browsers, just in case.

    Some additional information, in case it is helpful:

    SuiteCRM version: Version 7.11.10 Sugar Version 6.5.25 (Build 344)

    Downloaded add-on version: WebToModule5.3

    Can you provide more instructions regarding this line?: "do execute query for new table definition" ... I am not sure how to execute this task.

      Once you quick repair & rebuild. At bottom of the screen you will have a box like query window which will show you database mismatch. You will also have button called Execute. Do execute that query. If you are not sure about it. Feel free to contact us on Skype. Our Skype id is urdhvatech

      After the quick repair & rebuild, I do not see at the bottom of the screen a box like query window showing a database mismatch or a button called Execute.

      I am currently on Skype waiting for Urdhva to accept the invitation.
