If you are using WebToModule to generate a web form that capture data and creates Case in CRM. The webform itself should collect the information on who the data is of. Like, say, Name or email address of the person, along with the other information you may want to capture.
But as per our discussion, yesterday over google meet. The best solution for you is to use the Customer Portal solution, That will give you the proper way to manage the Case/Support ticket management.

can we ues LDAP authentication with Customer Portal ?

Hi bashayeralorf,
The portal will be used by your customers(Contact module). So login of customers will not require LDAP authentication.
The Customer Portal is for Support and Case solution. Which is used by your customers (Contacts). Given the username and password to the customers, they log in to the portal and create cases and do other self-service activities.
And the WebToModule is for gathering information without authentication, it is a publically available form, which you can use to get data in your CRM, Say, for example, Capturing Leads, Contact us form, etc.
