Hello Robert,
It can be achieved in another way. You just create a boolean(checkbox) field from a studio in the module for which you want to have this facility, then simply create a logic hook before_save and if that checkbox is checked or unchecked and update email address to opt-in/out.

Ok. kind of defeats the WebToModule concept. I have marketing users that have no idea how to do that. They know how to create forms in the module.
