Show/Hide logic does not seem to be working, Would appreciate some assistance please.

Actually, you would not be able to check the survey logic to show/hide questions in the preview survey page. You can only see the content of the form but would be unable to see the logic that you have added. The simplest way to check it is as follows:
1) Create a survey with adding logic in it.
2) Open the survey form using the open URL and check the logic of the questions that you have set.
3) By Opening, the survey form using the open URL will create a form transaction entry in the form transaction module for that survey and so you have to delete that entry after checking skip logic implementation.
