General Concepts
Diagram is drawn based on "Reports To" field in Contacts module so while create new contact you should be link contact to another contact who is manager of contact. For example: Account A have 2 contacts: - Sales Manager - Sales So while create record for Sales please link Sales to Sales Manager thought 'Reports To' field. The go to detail of Account A and select 'Organisation Chart' you will see diagrams here.
