#4407 - Create new contact screen not working

Hello,

We are running SuiteCRM Version 7.12.6 and when we enable this plugin, the create a new contact screen doesn't work - just a blank screen. Creating new accounts or leads works just fine. I have tried unchecking the options in the duplicate checker for contacts and still have the same issue.
