For Delete Icon Broken, Can you please check Delete Icon available in below path or not, /themes/SuiteP/images/delete_icon.png
If Delete Icon available then please check Icon having proper permission or not like 775 or 777.
As per your screenshots, It's look like folders and files doesn't have proper permission.
Please give recursive 775 or 777 permission to SuiteCRM folder.
If it's not working after giving the permission then please tell us which version of SuiteCRM you're using.
